Have you ever gotten that message when you boot tethered? You think that you screwed your device up and have to restore? Well, you did. JK This is how to fix that. You WILL be surprised by the answer to this.
Follow this 1 step:
1. At the last step when it says keep holding your home button for 15 seconds keep on holding it. Don't let go of the home button.
SURPRISED?!! Well, it worked for me. Might not work for some.
If that doesn't work then try this way:
If you use USB 3.0, use USB 2.0.

Still no work? Try this:
1. Boot tethered without pointing to the 5.0 firmware.
2. Point to 5.0.1 and boot tethered again.
3. Point back to 5.0 and boot tethered again.
4. Don't let go of the home button when you get to that 15 seconds step.
That pineapple should pop up. When it does you could let go of the home button.
There was more to this part, but I forgot. I wonder what else I did.
Not working? Turn your device off and try again. Hold the power button for 6 seconds if it doesn't turn off after 4 seconds. Ignore the first 3 seconds of the first step. Just hold home + power for 10 second and let go of the power button without letting go of the home button. If it still doesn't work then do that 3 seconds when your device is off.
Still not working? The dev blog provides 2 redsn0w b13. http://blog.iphone-dev.org/
Try those if my steps don't work. Be sure to run in administrator mode.

The newest versions of redsn0w only fixes MobileSubstrate bugs and some shitty launch thing and more crap that I'm too lazy to type down.
That exploit failed happens when you did a step wrong or it just happens...

The newest redsn0w still has more bugs than the older ones. When the unther was released some people still experienced that error and there were more errors.

I know what you mean now. I just tried to get the ipad baseband on an iphone 3gs and it crashed during the reflashing stage. Then I tried the previous version and it worked without any errors.
The dev Team used to make the updated versions better, but now it has become worse. lol

I found that on my Asus laptop it was the Asus USB QuickCharge (USBChargerPlus.exe) that was interfering. After killing that exe, the jailbreaking process worked.
If you have a different PC/Laptop, check to make sure there is no application that needs to monitor and interfere with normal USB operation.
